In vielen Hausarzt- und Facharztpraxen entsteht der Eindruck, dass die Organisation zuverlässig funktioniert. Termine werden vergeben, Patienten werden versorgt und medizinische Leistungen werden erbracht. Aus interner Sicht erscheint der Praxisbetrieb häufig stabil.
Gleichzeitig berichten Patienten jedoch immer wieder von anderen Erfahrungen. Sie erhalten unterschiedliche Auskünfte zu derselben Fragestellung, müssen mehrfach nachfragen, warten auf versprochene Rückrufe oder erleben unterschiedliche Aussagen verschiedener Mitarbeitender. Einzelne Vorfälle wirken dabei meist unbedeutend. Für den Patienten entsteht jedoch ein Gesamteindruck, der weit über das einzelne Ereignis hinausgeht.
Patienten bewerten selten die Organisation selbst. Sie erleben deren Auswirkungen. Genau deshalb setzt der Structural Continuity Index (SCI) an einer Perspektive an, die in vielen Praxisanalysen bislang nur unzureichend berücksichtigt wird.
